General (non year-specific) Forms

Prior to accessing forms, read their brief descriptions to determine if you need to complete them. Not all forms are required.

Note: Most of our forms are interactive, meaning you can key in the required information while online. After printing your form, be sure to sign it before faxing or mailing it to Enrollment Services.

NEW! You can fill out and submit some forms online. The link will take you to Regis University INsite, where you need to log in. Once logged in, your form will appear. Fill it out and submit. Do not mail or fax these forms.

Colorado Residency Form
Use this form to qualify as a Colorado resident if this information is requested by the Financial Aid office.

Book Voucher Instructions and Form - SUBMIT ONLINE.
Use this form if you have financial aid and you want to use some of your funds to buy books.

Authorization to Request COF Stipends
This form is used to either authorize or rescind authorization for releasing the COF stipends to your student account.

Change of Address, E-mail Address, or Phone Number

Credit Balance Hold
If a credit balance exists on your account and you would prefer to leave it there for future any charges rather than receive a refund, fill out this form.

The Free Application for Federal Student Aid (FAFSA) is available at www.fafsa.ed.gov. Once completed and submitted, this application makes you eligible to apply for federal, state and institutional grants, loans, student employment, and scholarships.

2009-2010 Forms

For Summer 2009, Fall 2009, and Spring 2010

Student Employment Application Form: Use this form if you are interested in working on campus. If you fill out the FAFSA, you do not need to fill out this form. This form does not apply to summer employment.

FAFSA Income Appeal 2009-10
Use this form to report reductions in family income that are not reflected on the original Free Application for Federal Student Aid (FAFSA) form.

Institutional Verification Form - New Students
Use this form if you are a new student and do not yet have a RegisNet account, or if you prefer not to submit electronically, and your 2009-2010 SAR indicates you are selected for verification and/or the Financial Aid office has requested this form.

Institutional Verification Form - SUBMIT ONLINE.
Use this form if your 2009-2010 Student Aid Report indicates that you are selected for verification or the Financial Aid office has requested this form. Please Note: When you click this link, you will be connected to the INsite login page. Please login using your RegisNet account login and complete the IVF. When you click the SUBMIT button at the end, the form will be submitted on-line directly.

Teach Grant Application
For students who commit to 1) teach full-time, 2) in high-need subject areas, 3) for at least four years, 4) at schools that serve students from low-income families. Graduate students may be eligible for $4,000 per year ($8,000 total) with this same commitment to teach.
