Prior to accessing forms, read their brief descriptions to determine if you need to complete them. Not all forms are required.
Note: Most of these forms are interactive, meaning you can key in the required information while online. After printing your form, be sure to sign it before faxing or mailing it to Enrollment Services.
Enrollment Services is working on creating completely automated forms. Any forms designated "NEW INsite form" can be submitted online rather than mailed or faxed. You must use Adobe Acrobat to download these forms.
Authorization to Attend Closed/Restricted Class
In order to attend a course that has been closed, a student must obtain permission from the instructor. Bring this completed and signed form to Enrollment Services, Fourth floor, Main Hall.
Certification of Dependency
In accordance with Federal regulations, the Family Educational Rights and Privacy Act of 1974 (FERPA), as amended, governs a university’s ability to release confidential information in a student’s education records.
According to the Act, confidential education records may not be released to parents/guardians without consent of the student unless the student meets the definition of “DEPENDENT” as defined by the Internal Revenue Code of 1954, Section 152.
Parents/Guardians may obtain confidential education records by returning this signed form or by providing a signed photocopy of their most recent income tax return showing the student as a dependent. Any financial data on the return may be crossed out.
Change of Address
Fill out this form if you are changing either your local or permanent address. If you are a Regis College student changing only your on-campus address, fax this form to Student Life at 303-964-5530. If you have graduated, fax this form to the alumni office at 303-964-5423 or go to Update My Information. If you are an employee and a student, you need to fill out only the online Human Resources Change of Address form.
Change of Name.
Regis University maintains an educational record for each student who is or has been enrolled at the University. These records are maintained under the name on your application for admission to Regis.
If you want to change your name on educational records, please submit a Change of Name Affidavit. These forms are also available in the Enrollment Services office.
Duplicate Diploma
A duplicate diploma may be ordered if your original diploma has been lost, damaged, or stolen. There is a $25 charge, and your new diploma will be printed on the paper currently in use for diplomas.
Enrollment-Degree Verification Request
The Office of Academic Records and Registration processes all official enrollment or degree verification requests. There is no charge for this service.
If a loan or insurance agency is requesting verification on a specific form, please complete their form before sending it to the Registrar for official verification.
Please note: We cannot process enrollment verification for the current semester until after the add/drop period.
Non-Release of Directory Information
Students may request that their name and all other information pertaining to their attendance at Regis University be withheld from the University Student Directory and Commencement Programs.
Pass/No Pass Request
The PASS/No PASS (P/NP) option is available to all students who have completed 30 semester hours of graded courses. Transfer students must complete 20 semester hours of graded courses at Regis University.
Enrollment Services - Registrar must receive this request before 3:30 p.m. of the last day of the Add/Drop period.
The P/NP option may be exercised for only one course each semester and only two courses (total) of the CORE studies requirements. The P/NP option cannot be used for courses in a student’s major or minor.
The maximum number of hours that may be taken on a P/NP basis is 18 semester hours.
Personal Data Update
This form is used to correct the personal information that was provided on your application to the University or to provide information that was missing.
Student Release of Education Records
The Family Educational Rights and Privacy Act of 1974 (FERPA governs a university’s ability to release confidential information in a student’s education records. Confidential education records may be released only with the permission of the student. By signing this release form, the student gives Regis University permission to release his/her confidential information to the designated person and/or company.
Transcript Request Form
Transcripts may be requested by fax, mail, or in person. Due to federal student privacy requirements, requests by phone or e-mail cannot be accepted. If your transcript will be picked up by someone else, please provide a written release with your signature. Transcript requests are processed only after all outstanding financial obligations to Regis University are met.

Regis College Total Withdrawal/Leave of Absence Leave of Absence status is available to undergraduate students who plan to leave Regis College for one or two semesters and then return. Total Withdrawal may be requested by a student through the withdrawal deadline for the semester. More detailed information is available on the form.
