Forms and Publications

Many of our forms can now be submitted online! You must be logged in to your Regis account to submit these online forms. If you are not already logged in to your Regis account when you click the links below, you will be prompted to log in. Some online forms require parental information or a parent's signature. If this is the case, parental information will be requested on the form and an email will be sent to the parent requesting that they create an account, log in and complete their portion of the online form.

All paper forms must be downloaded using Adobe Acrobat Reader. Please download the free Adobe Reader application before filling out our forms. If you are still having difficulties, call ITS at 303.458.4050. After completing your paper form, be sure to sign it before sending it to Financial Aid.

Many Student Accounts Forms, including the Check Reissuance Request, Credit Balance Hold form and Student Book Voucher form, can be found by going to the Ranger Portal, logging in and going to the Student Finance menu.
