Certification of Dependency
In accordance with Federal regulations, the Family Educational Rights and Privacy Act of 1974 (FERPA), as amended, governs a university’s ability to release confidential information in a student’s education records. According to the Act, confidential education records may not be released to parents/guardians without the consent of the student unless the student meets the definition of “DEPENDENT” as defined by the Internal Revenue Code of 1954, Section 152. Parents or Guardians may obtain confidential education records by returning this signed form or by providing a signed photocopy of their most recent income tax return showing the student as a dependent. Any financial data on the return may be crossed out.
Change of Address
Students may update their address information through the Ranger Portal. A login is required.
Change of Name Affidavit
Regis University maintains an educational record for each student who is or has been enrolled at the University. These records are maintained under the name of your application for admission to Regis. Please submit a Change of Name Affidavit if you want to change your name on educational records.
Duplicate Diploma Request
A duplicate diploma may be ordered if your original diploma has been lost, damaged or stolen. There is a $25 charge, and your new diploma will be printed on the paper currently in use for diplomas.
Emergency Contact Form
Students may update their emergency contact information through the Ranger Portal. A login is required.
Enrollment/Degree Verification Request
The Office of the Registrar processes all official enrollment or degree verification requests. There is no charge for this service. If a loan or insurance agency is requesting verification on a specific form, please complete their form before sending it to the Office of the Registrar for official verification. Please note: We cannot process enrollment verification for the current semester until after the add/drop period.
Family Information Update
This form is used to correct the family information that was provided on your application to the University or to provide information that was missing.
Graduation Application: Degree
Use this form to submit your application for graduation upon completion of your degree program.
Graduation Application: Certificate
Use this form to submit your application for graduation upon completion of your certificate program.
Individual Course Withdrawal Form
Students who withdraw from courses after the add/drop period may qualify for a partial credit of tuition. Credits are calculated at rates established for each week. Withdrawal procedures vary depending on the student’s college and level. Please be aware that course withdrawal amounts will vary by term weeks. See the Academic Catalog for the most up-to-date information. You will fill out and submit the form online which will date and time stamp the form. Do not mail or fax this form.
Non-Release of Directory Information
Students may request that their name and all other information pertaining to their attendance at Regis University be withheld from the University Student Directory and Commencement Programs.
Personal Data Update
This form is used to correct the personal information that was provided on your application to the University or to provide information that was missing.
Preferred Tuition Form
Regis University offers preferred tuition rates to students who are licensed teachers, paraprofessionals or employed by an organization with which the University has an active partnership. Before the discounted rates can be applied, we must receive all required documentation. A list of the documentation including the Preferred Tuition Partnership Form is provided within the link.
Release of Records to Third Parties
The Family Educational Rights and Privacy Act of 1974 (FERPA) governs a university’s ability to release confidential information in a student’s education records. Confidential education records may be released only with the permission of the student. By signing this release form, the student gives Regis University permission to release his/her confidential information to the designated person and/or company.
Student Release of Education Records to Third Parties NOT Parents or Guardians
The Family Educational Rights and Privacy Act of 1974 (FERPA) governs a university’s ability to release confidential information in a student’s education records. Confidential education records may be released only with the permission of the student. Parent and guardian proxy access may be given by the student through the Ranger Portal. A login is required.
Transcript Request Form
If you are unable to order transcripts online, you can also request a transcript using the Transcript Request Form. You must print and sign the form before you mail or submit the form in person. The form must be submitted along with a check or money order made out to Regis University ($14.00 per transcript). Due to federal student privacy requirements, requests by phone or email cannot be accepted. If someone else is picking up your transcript, you must provide a written release with your signature along with the transcript request form.
